We are seeking a skilled, motivated, and dynamic Nurse with a passion for high quality care and service development.
You will be responsible for managing your own clinical caseload of community patients as well as supporting the HCA/rehabilitation assistants with training, supervision, and joint sessions.
The Rehabilitation And Frailty Team (RAFT) provides daily input for up to 21 days in the patient’s own home. The service hours are 8am – 8pm with a care element added 7am – 10pm, 365 days a year. You will be working occasional weekends and bank holidays on a rota basis.

We continuously prioritise our caseload to be responsive to the changing daily needs of our service users, communicating closely with other service providers to avoid admission where it is safe to do so and providing onward referrals to other community services including health and social care. As part of the Rehabilitation And Frailty Team, we see a wide variety of patients including:
