Job Opportunity: Band 6 Team Leader in Discharge Lounge
This is an exciting opportunity for development in the discharge lounge. We are a dynamic, knowledgeable, and adaptable team promoting safe same-day discharges for all patients, involving many multidisciplinary team members, families, and community services.

We operate in a fast-paced environment, working closely together to provide an excellent service, putting the patient at the forefront of our plans. By enabling earlier discharges, we act as a safety net, allowing wards the time to care for new clients.

Key Responsibilities:

  • Liaise with patients, families, care providers, transport teams, and more, both verbally and electronically, to ensure a safe discharge with all follow-up care and plans in place.
  • Work independently and as part of a team to promote safe discharges.
  • As the Band 6, lead the team from the front, engaging with team members to enhance their skills and learning.
  • Liaise with all wards to promote engagement and the service.
  • Assist with the daily running of the unit and stand in for the Band 7 when they are away.

We employ more than 6,500 staff who deliver compassionate care from our two main hospitals, Calderdale Royal Hospital and Huddersfield Royal Infirmary, as well as in community sites, health centres, and patients’ homes. We are incredibly proud to have almost 150 volunteers at CHFT.
