About The Role
Abbeycare is a trusted provider of supported living, domiciliary, palliative and end-of-life homecare services. This role is specifically for our domiciliary palliative care role, supporting individuals with life limiting conditions to remain safe, comfortable, and cared for in their own homes. We deliver holistic support, including personal care, pain management assistance, emotional support, and companionship always delivered with dignity, sensitivity, and respect.

Role Overview
As a Domiciliary Palliative Care Worker, you will support individuals approaching the final stages of life by delivering high quality, person centred care within their own homes. Your role will include assisting with personal care, helping manage symptoms in line with clinical instructions, providing emotional reassurance, and supporting families during a sensitive and challenging time.
You will work alongside healthcare professionals (district nurses, GP teams, hospice staff) to ensure seamless, dignified care that enhances comfort and quality of life.

Key Responsibilities

  • Personal and Intimate Care: Provide sensitive support with washing, dressing, continence care, repositioning, and comfort measures.
  • Symptom & Medication Support: Follow palliative care plans, assist with safe medication prompts, and observe and report changes in symptoms to the relevant healthcare professionals.
  • Comfort & Emotional Support: Offer reassurance, companionship, and a calming presence to individuals and their families.
  • Dignity & Respect: Ensure every aspect of care is delivered with compassion, privacy, and respect for personal wishes, cultural beliefs, and family dynamics.
  • End-of-Life Support: Assist in creating a peaceful, person-centred environment, supporting individuals in maintaining comfort and dignity in their final days.
  • Nutrition & Hydration Support: Assist with safe food and drink preparation, or with comfort feeding as appropriate to the care plan.
  • Home Environment: Help maintain a clean, safe, and comfortable home setting, ensuring equipment is used safely.
  • Family Liaison: Work alongside families, offering guidance, support, and reassurance while respecting boundaries.
  • Communication & Reporting: Document changes, follow care plans carefully, and communicate concerns promptly to the Service Manager and clinical teams.
  • Professional Collaboration: Work with district nurses, GPs, and hospice teams to support coordinated care.

Role Type: Part Time and Full Time positions available

Hours: 16-36 hours (Bank contracts also available)

Shift Patterns: Vary - includes alternate weekends and some evening or night support

What You’ll Bring

  • A kind, patient, and compassionate attitude
  • Experience in a care or support role (palliative care experience is desirable but full training is provided)
  • Strong communication and emotional resilience, especially during sensitive situations
  • Understanding of person-centred and dignity-focused care
  • Ability to work calmly and independently
  • Flexibility to support individuals whose needs may change quickly
  • Willingness to work with families and healthcare professionals
  • NVQ Level 2 or 3 in Health and Social Care (or equivalent) - desirable
  • A full UK driving licence - Essential - Mileage allowance paid.
  • Availability for weekend work.
