Band 3 HCSW - Full time - Permanent

The post holder will be based within our established South Stoke Community CAMHS Team and will support the team in delivering on timely assessment and evidenced based intervention.

The Successful Candidate Must

  • Have experience of working with people with health needs.
  • Have experience of working in a multi-disciplinary team.
  • Be able to work autonomously.

Key Responsibilities:

  • Provide support and spend therapeutic time with an allocated group of children, young people and families.

  • Promote and support the physical health, comfort, wellbeing and recovery of children, young people and families, and assist them in the recovery of their difficulties.

  • Assist Registered Professionals in assessments and care planning/implementation and evaluation of individualised care plans.

  • Provide routine advice on areas of knowledge/expertise, relevant to the audience and topic area (i.e., patients, carers, complexity).

  • Liaise with partner services and organisations.

  • Prepare/complete routine documents, including letters, reports, presentations and forms (e.g., photocopying, filing).

  • Communicate with children, young people and families, all members of the multi-disciplinary team, and staff in external agencies using a variety of methods including Information Technology.

  • Regularly report and effectively communicate the health status and care aims of children, young people and families in care records and verbal reports to Registered Health Professional staff members and other members of the multi-disciplinary team.

  • Relate appropriately with children and young people, carers, colleagues and others at all times, maintaining professional boundaries.

  • Ensure that own actions support the equality, diversity, rights and responsibilities of individuals.

  • Identify, minimize and report risks of danger, harm and abuse to senior colleagues.

  • Minimize risks to self and others by undertaking safe working practices.

  • Work within agreed and established Trust policy and operational procedures.

  • Work with other Trust services to promote seamless service-user person-centred care from referral/admission to transfer and discharge on identified care pathways.

Due to the increase to the salary threshold to £25,000 per annum, after 9 April entry-level Band 3 roles will no longer be eligible for international sponsorship. Therefore, should you require sponsorship for this post, you must have had 3 years continuous employment at Band 3 or higher within the NHS in a similar or equivalent role in order to be eligible to apply.
