Care Coordinator
Location: Caremark East Riding, Hessle, HU13 9PB
Salary: £24,251.52 per annum
Hours: Full-time, Monday to Friday (with one weekend in four)
Holiday: 28 days per year
Driving licence and access to a vehicle required

About the role
We are seeking an experienced and dedicated Care Coordinator to join our professional and compassionate team. This is a pivotal position within our organisation, responsible for coordinating and overseeing the delivery of high-quality, person-centred care across your designated area.

You will work closely with field-based supervisors and care staff to ensure that every service user receives tailored support that promotes independence, dignity, and wellbeing.

Key responsibilities

  • Complete Rota's ensuring our support is focused on individualized and person-centered care
  • Developing care plans and rearranging them as necessary when difficulties arise liaising with care staff
  • Be part of the rolling on-call team Rota, providing out of hours if necessary
  • Pre-empt problems and manage day-to-day issues whilst identifying and offering solutions to ensure continued support to service users
  • Interview new care staff and work with our onboarding and compliance teams to ensure all pre-employment checks are in place
  • Ensure that staff deliver high-quality and personalised care to service users.
  • Recruit, train, and supervise staff and communicate the needs of service users and care teams
  • Ensure health and safety compliance, undertake service user visits, check on the care they've received and document it accordingly
  • Liaise with local community organisations and authorities, maintaining good relationships with all stakeholders
  • Ensure that all activities such as personal care and medication administration meet the required standards
  • Prepare reports related to the progress of service users
  • Plan and utilise resources, staying updated with care standards and knowledge to ensure that service users receive the best care

About you

  • Previous experience as a Care Coordinator (essential).
  • NVQ Level 2/3 (above or equivalent) or willingness to work towards it.
  • Strong communication and interpersonal skills with a professional, empathetic approach.
  • Proven ability to manage staff rotas and utilise rota management software effectively.
  • Competent IT skills, including proficiency in Microsoft Office.
  • Organised, methodical, and detail-oriented, with the ability to work well under pressure.
  • A full UK driving licence and access to a vehicle are essential due to the nature of the role.
