Job Overview
An exciting opportunity has become available to join the Acute Medicine team at PAH as an Advanced Clinical Practitioner. This position is for a qualified ACP with experience in Acute Medicine; however, a trainee position would be considered for the right candidate at Band 7 who would be expected to undertake an MSC in Advanced Practice via the apprenticeship route commencing in January 2026.

As part of a multi-professional team, led by the lead Acute Medical consultant, you will practice autonomously within an acute setting to provide patient-centred quality care. This will encompass the skills of assessment, examination, diagnosis, and treatment within an agreed scope of practice within Acute Medicine (AAU, ED, and SDEC) whilst being accountable and self-directed in line with the relevant code of professional conduct.

You will support efficient and patient-focused service and will ensure safe referral and discharge of patients with undifferentiated and undiagnosed presentations in any area of the Acute Medicine department. 80% of the post holder’s role will be clinical and 20% related to clinical audit, teaching, self-development, development of the service, and research.

Main Duties of the Job

  • Assess and prioritize patients attending or referred to acute medicine and the Same day Emergency Care department (SDEC) with undifferentiated health conditions.
  • Provide a level of advanced and comprehensive assessment based on a specialist body of knowledge.
  • Direct responsibility for the management of patients within own case load and supervision/development of junior members of the multidisciplinary team.
  • To appropriately assess, examine, investigate, diagnose, and treat patients, resulting in the safe management and appropriate referral or discharge of patients with undifferentiated and undiagnosed presentations.
  • To contribute to the development and improvement of systems and processes that facilitate patient flow.

Detailed Job Description and Main Responsibilities

  • Assess and prioritise patients attending the Acute department with undifferentiated health conditions.
  • Work in all areas across the Acute Department, for example; Rapid assessment and treatment, minors, clinical decision unit, majors, and resuscitation.
  • Provide a level of advanced and comprehensive assessment based on a specialist body of knowledge.
  • Direct responsibility for the management of patients within own case load and supervision/development of junior members of the multidisciplinary team.
  • To appropriately assess, examine, investigate, diagnose, and treat patients, resulting in the safe management and appropriate referral or discharge of patients with undifferentiated and undiagnosed presentations.
  • Act as a role model.
  • Support medical, nursing colleagues and other allied health professionals within their roles.
  • Maintain a professional portfolio (Royal College of Emergency Medicine). Maintain Nursing and Midwifery Council PIN.
  • Attend meetings relevant to service delivery and development and influence decisions regarding service delivery.
