Job Overview
Band 5 nurses work closely with the people who use our services, their families, friends, and carers, playing an important contribution in how people experience our services.

Band 5 nurses are expected to be kind and responsive, yet professional and informative, contributing to the quality of the services we provide by:

  • Keeping the people who use our services as safe as possible through sound clinical skills and effective risk assessments.
  • Ensuring the best possible clinical outcomes by using up-to-date skills and adhering to evidence-based policies and procedures.
  • Ensuring that people using our services have a good experience by respecting, empowering, and working in partnership with them throughout the care planning process.

Main duties of the job
An opportunity for a Staff Nurse wishing to work part-time hours to join our friendly Twilight Team. Our aim is to be the best we can, offering excellent person-centred, safe, and evidence-based care to those who access our services. Our Twilight District Nursing Team delivers "out of hours care" to people in their own homes and/or designated buildings. The team is made up of both staff nurses and healthcare support workers who provide an out-of-hours district nursing service.

You must hold a relevant Nursing Qualification with significant experience as a staff nurse. You must be a car driver and have access to a car for use at work with business insurance.

You will:

  • Perform joint visits to provide nursing care to patients allocated by a senior staff member.
  • Take responsibility for the assessment, management, and evaluation of evidence-based nursing care to patients in various community settings, promoting effective teamwork within the wider Health Care Team.

Part Time: 4.5 hour shifts, with a total of 27.0 hours available to do job share between more than one member of staff.

The service operates seven days a week between the hours of 16:30 - 00:30.
The post consists of 4.5 hour shifts. The shift starts at either 18:00 until 22:30 hrs or 19:30 until 00:00 hrs depending on service needs. The contracted hours will be 27.0 hours in total available to do job share between more than one member of staff.

Essential criteria:

  • Demonstrable experience of clinical leadership.
  • Clinical experience of caring for people with cognitive impairment.
  • Experience of mentoring students.

Education And Qualifications
Essential criteria:

  • Registered NMC nurse (Adult) or awaiting pin, diploma, or degree.
  • Numeracy skills required for the safe administration of medicines.
  • Evidence of ongoing relevant CPD.
  • Willingness to undertake further studies as required as part of this role.
    Desirable criteria:
  • Community experience as a student or in a substantive role.

Skills and knowledge
Essential criteria:

  • Able to communicate with the multi-disciplinary team effectively, using handover, face-to-face communication, and by telephone, as well as writing letters and reports.
  • Able to maintain electronic patient records and have a working knowledge of Microsoft Office.
  • Care planning: being able to assess patients, develop, implement, and evaluate a care plan.
  • Empowering and working in partnership.
  • Safeguarding and advocacy.
  • Understanding how the Mental Capacity Act and Deprivation of Liberty Safeguards impact on their work and the people they are working with.
  • Understanding the concept of quality and their role in improving safety, clinical outcomes, and the patient experience.
  • Knowledge of recent developments in health & social care.
  • Able to pass on urgent client information to District Nurse/ Team Lead.
  • Able to organize own workload without direct supervision.
  • Act as a mentor to HCAs, Nursing Associates, and students.
  • Understanding and appreciation of diversity and equality in practice.
