Community Nurse

We are looking for a Community Nurse to work in and around the St Neots/A1 Locality. You will be fully supported to gain new skills and experience needed to deliver high-quality nursing care to patients in their own home or residential setting.

As a community nurse, we will develop your wound care, palliative care, and long-term conditions knowledge and skills; supporting you to become an expert in assessment and care-planning to deliver quality care that is caring and compassionate.

Please note: for this role you will be required to travel independently around the county meeting strict time deadlines. You will need to hold a full UK driving licence and have use of a vehicle. You cannot use public transport for this role as this is not a reliable form of transport and will not allow you to meet service needs. We Are Looking For People Who

• Have experience of working in the Community
• Are educated to RGN level 2
• Are able to demonstrate a commitment to high-quality care and service provision
• Have the ability to work flexibly and as part of a team
• Have the ability to assess and formulate care plans
• Are able to teach and support student Nurses and Health Care Assistants
• Have enhanced communication and interpersonal skills

You Will Be Responsible For

• Keeping the patient at the centre of care, ensuring patient and carer participation in decision making
• Providing a high standard of care
• Providing a high standard of nursing intervention within a patient's own home, with access to Specialist and Advanced Practitioners when required
• Working as part of a 24/7 service and multidisciplinary team
