We have a full-time Respiratory Specialist Nurse role with the opportunity to join the Integrated COPD at the University Hospital Southampton NHS Foundation Trust (UHS).

Detailed Job Description And Main Responsibilities
The Integrated COPD Team provides specialist evidenced based care and support for patients with COPD within the acute hospital and ongoing support, monitoring and education within their homes to promote independence to manage their disease within primary care.
We operate 8am-6pm 7 days a week with biweekly telephone On calls. We cover the greater Southampton area and are hugely proud of the rapport we have built with our patients and the trust they have in us.

  • Establish contact with patients/carers from the time of initial referral or identification to provide specialist nursing advice and practical, psychosocial support.
  • Communicate to patients in a way that empowers them to make informed choices about their health and health care.
  • Manage patient care to ensure patients receive care that works in partnership with other healthcare professionals including referrals to alternative care pathways and treatments as clinically indicated.

What We’re Looking For
As a Respiratory Specialist Nurse within the Integrated COPD Team, you will be expected to have excellent communication skills, promote the delivery of a high standard of patient-centred care and work between organisational boundaries. You will be expected to work across different organisations with the support of a Senior Sister.
You will analyse, monitor and implement actions that ensure the service delivery is safe and effective in line with the Trust values. You will be expected to be a coach to our patients, their families, and hospital staff for better understanding of COPD.
The post holder will require a flexible approach to working patterns and will be required to work to a rota that includes weekends and bank holidays. The post holder will also need to be flexible and cover the community non-invasive ventilation on call service out of hours (telephone only).
The post holder will need to be adaptable and flexible and work in varied environments, including the hospital, home visits and on multiple sites.

Qualifications, Knowledge And Experience
Essential criteria

  • Registered nurse on relevant part of the NMC register
  • Degree qualification, 120 credits at L6 or possess an equivalent knowledge to be assessed through evidence of relevant specialist training, experience or portfolio
  • Significant post registration experience / qualification / training, including experience and knowledge within relevant speciality
  • Completion of all Band 5 (preceptorship) competencies
  • Higher Education Institute approved course in supporting learners in practice or evidence assessed to be of equivalent experience, demonstrating support to a range of learners in practice. Provide evidence of both supporting a range of learners in practice and supporting colleagues to promote effective learning in practice
  • Extensive post registration experience within specialist area.
  • Knowledge of current issues within the acute health service

Desirable criteria

  • Change Management, clinical practice development.
