We are seeking a new Evening and Night Nurse to work in an already established team. We have vacancies in both the North/South Nottinghamshire team. We cover:
We work closely with our Urgent Community Response teams to ensure the right clinician attends the right urgent response during the evening and night-time, maintaining safety and delivering nursing care when needed.
Our service is fast-paced during the out-of-hours period with a 2–4-hour response time, covering the nursing elements within the UCR clinical conditions. This includes urgent end-of-life care, continence care, catheter care, and wound care needs overnight.
As the service transforms, we are looking for dynamic, autonomous practitioners with excellent communication skills who can be confident, organised, adaptable, reliable, and approachable. You’ll be working independently in patients’ homes, collaborating with UCR services and the out-of-hours GP provider to ensure the right team attends to the patient to fulfil their urgent nursing needs.
Our team is mixed with nurses and community support workers, working in partnership with the community nursing day teams and UCR to provide a seamless nursing service to patients from 6pm to 8am, putting the patient at the centre of their care and providing effective patient care.
The objective of our evening and night service is to deliver good clinical outcomes for patients in partnership with day services and UCR across the whole health community. We aim to co-ordinate a shared care approach for patients across primary and community care, avoiding duplication and effectively managing resources to provide timely access. The main focus is admission avoidance, proactively identifying patients at risk of hospital admissions or readmissions, and providing support and care to patients and their families during the out-of-hours period.
